Dear All,

1. By submitting this form, you consent to the processing of your personal data for the purpose of providing you with information about the services delivered by Galach Consulting Sp. z o.o., with its registered office in Warsaw (02-512), at ul. Puławska 14, hereinafter referred to as the Company. Your personal data will be processed by the Company, which is the personal data controller.
2. You may withdraw your consent at any time by informing the Company via email at info@galach.pl, without affecting the lawfulness of processing based on consent before its withdrawal. Please include information about the withdrawal of consent in the body of the email.
3. Your personal data will be processed by the Company pursuant to Article 6(1)(a) of the General Data Protection Regulation (GDPR).
4. The recipients of your personal data are:
   • The Management Board, employees, and associates of the Company, in accordance with the scope of their duties.
   • Entities entrusted by the Company with the processing of personal data in relation to the services provided by these entities to the Company.
5. Your personal data are not transferred to third countries or international organizations.
6. You have the right to request from the personal data controller: access to your personal data, rectification, data erasure, restriction of data processing, and data portability. You have the right to object to the processing of your data by the Company.
7. Your personal data will not be processed in automated decision-making, including profiling.
8. Your personal data will be processed by the Company until you withdraw your consent, but no longer than for a period of one year from the last contact with you.
9. Personal data are subject to immediate erasure upon request of the data subject, provided that the conditions specified in Article 17 of the General Data Protection Regulation (GDPR) are met.
10. You have the right to lodge a complaint with the supervisory authority responsible for personal data protection.
11. Providing your personal data is voluntary, but necessary to contact you and provide information about the services provided by the Company.
